Spay|Neuter Terms and Conditions

Thank you for being a responsible pet owner and helping to control the stray animal population in our community! A lot of information is below, but it is important to read it all thoroughly to keep our program up and running smoothly.     


Step 1 -   Complete your payment as indicated in the email or letter you received.

Step 2 - Schedule your pet's appointment with Fayette Veterinary Clinic by calling 205-932-4545, be sure you tell them when you call that you are going through the PAWS program. Procedure payments are due prior to us approving your appointment with the vet. Only once we receive your payment will we notify the vet's office that you're approved to use our program. 

 

**NOTE: IF YOU FAIL TO REMIND THE VET AT DROP OFF THE DAY OF YOUR APPOINTMENT THAT YOUR APPOINTMENT IS THROUGH PAWS AND YOU ARE CHARGED THE VET DOES NOT HAVE TO REFUND YOUR MONEY. THIS IS PART OF OUR PROGRAM AND IT IS YOUR DUTY TO REMIND THEM THE DAY OF THAT YOUR THROUGH THE PAWS PROGRAM. WE SUGGEST TAKING A COPY OF YOUR PAID RECEIPT/APPLICATION WITH YOU TO PROVIDE TO THEM AT DROP OFF TO BE ON THE SAFE SIDE AND AVOID CONFUSION. WE APPRECIATE THE VET'S OFFICE AND THEIR ASSISTANCE WITH OUR PROGRAM AND WILL DO WHATEVER IS NEED TO MAKE THE PROCESS EASY FOR THEM AND THEIR STAFF. THANK YOU FOR UNDERSTANDING.**


*Rabies vaccine and microchips are now required within the program, if you CAN provide proof of an up-to-date rabies vaccination certificate (within the last 12 months from the date of the appointment) the cost of the rabies vaccination will be deducted from your amount due. We CANNOT deduct the amount due without proof of vaccination in hand first. It can be uploaded when completing the Animal Information Sheet.


The microchips are mandatory unless your pet is already chipped. If you indicate they are chipped already on the application the vet will scan them at their appointment, and if no microchip is detected they will receive one during their spay/neuter procedure. This is being done as part of our animal management for our county so that if the pet being fixed is ever found as a stray, etc. we can scan it and help identify who the owner is.


OUR PROGRAM DOES NOT COVER ANY OTHER MEDICAL SERVICES EXCEPT FOR THE RABIES, SPAY/NEUTER, AND MICROCHIP. Any other services you'd like would need to be discussed with the vet when you call to schedule your appointment AND are paid for separate and direct to the vet.*


**Applications that we've not received payment on within 60 days of their approval notification will be voided. If you schedule your appointment without us receiving your payment, your appointment can still be used but the cost will NOT be our program cost and will be determined by the vet and due to them the day of service.  ONCE YOU MAKE YOUR PAYMENT TO US, ANY CANCELED APPOINTMENTS THAT ARE NOT RESCHEDULED AT LEAST 24 HRS IN ADVANCE OR ARE MISSED/NO-SHOWS ARE ALL NON-REFUNDABLE AND YOUR PAYMENT IS CONSIDERED A DONATION. **


The Vet reserves the right to refuse service to any pet if vet nor staff have been told prior to the day of the appointment of the possibility of complications arising from the A - C below. If your pet has the potential to fall into any one of the below categories, contact us immediately so we can discuss with the vet on how to appropriately handle your pet in particular so we can get their appointment scheduled and handled appropriately and safely. Your payment will not be refunded if you have to reschedule due to failure to notify us of your pet falling into one of the below categories at least 7 business days prior to their appointment date and if you choose to reschedule it will be at your own expense at the veterinarian's regular service rate. 

A. Kennel Crazy - If your pet is NOT used to being in a crate or kennel, you need to make your best effort to get them comfortable with this BEFORE the day of their appointment. When they're at the vet's office they'll be in a kennel or crate for holding and they do not need to damage anything or injure themselves or someone else. You must get your pet accustomed to staying in a crate or kennel from now until your appointment date so any foreseeable problems with your pet being kenneled  while at the vet's office can be appropriately prepared for. We will not be responsible for any damage or injuries your pet may cause if they are "kennel crazy".  The vet can hold you liable for their damages if they were not warned of the risk prior to your appointment. We may have a crate you can borrow if needed, just ask. If they are not comfortable with a crate or kennel at all you may be required to remain with them until they've been sedated but you have to let us know this ahead of time so it can be discussed with the vet for appropriate handling prior to the day of your appointment. 

    B. Unsocialized - If your pet is NOT used to strangers or other people and has ANY possibility to bite for any reason you will need to contact us immediately so we can discuss with the vet on how to appropriately handle your pet in particular so we can get  their appointment scheduled. You may be required to muzzle them and/or remain with them until they've been sedated.

  C. Illness/Infections - If your pet is exhibiting any signs of sickness (diarrhea, lethargy, vomiting, coughing, wheezing, sneezing, etc.) or has any sort of bacterial or fungal infections on the skin or within the body, they CANNOT be put under aesthesia for the procedure. They will be treated and clear for their condition before procedure can be re-scheduled. Should you take in a pet exhibiting any of signs of this you will be asked to return to the clinic immediately to pick up your pet and have the appointment  rescheduled at your own expense.


DAY/NIGHT BEFORE SCHEDULED APPOINTMENTS: We highly recommend you contain your pet some way the day/night before their appointment, we do have a limited number of crates that can be borrowed if need. You would need to make sure your pet does not have any food after midnight the night before their scheduled procedure.

*Additional fees may be incurred if there are any unforeseen complications or (for females) if they're in heat or currently pregnant. In heat females may have an extra charge that is the owner's responsibility to pay. No financial assistance is applied to additional charges for being in heat. It is requested that in heat females be scheduled 2 weeks after the end of their heat cycle. ONCE YOU MAKE YOUR PAYMENT TO US, ANY CANCELED APPOINTMENTS THAT ARE NOT RESCHEDULED, MISSED OR FORFEITED APPOINTMENTS ARE NON-REFUNDABLE AND YOUR PAYMENT IS CONSIDERED A DONATION.*

!IMPORTANT! If your pet did not have a microchip prior to their appointment, upon pick up of your pet from their appointment the day of the procedure you will be given a small information card to register your pet's new microchip. It is completely FREE to register. You will need to complete registration within 3 days from picking up your pet, it's quick and takes less 2 minutes at no cost to you. You can also see instructions to register - Click Here to Register your Pet's Microchip. Microchips are NOT a GPS device and cannot be used to track your pet's location. The microchip is a unique identifier that uses radio-frequency identification (RFID) technology to transmit a number when scanned by animal shelters, vets offices, animal control, etc.. The number is linked to a database that contains the owner's contact information, which can help reunite the pet with its owner if it's lost or stolen.  


Release of Liability

You hereby release Lamar County Humane Society|PAWS and the veterinary clinic where your pet's procedure is scheduled (hereafter referred to as "Veterinary Clinic"), their volunteers, employees, agents, and representatives from any liability for damage or injury caused by my pet(s) while on the premises of the veterinary clinic. I understand that Veterinary Clinic takes all reasonable precautions to ensure the safety and well-being of animals while they are under their care, accidents and unforeseen events may occur. By submitting this form I acknowledge that I am solely responsible for any actions or behaviors of my pet(s) while at Veterinary Clinic, and I agree to indemnify and hold harmless Lamar County Humane Society|PAWS and Veterinary Clinic, their volunteers, employees, agents, and representatives from any claims, damages, losses, or expenses arising from such actions or behaviors. I further understand that the Veterinary Clinic reserves the right to refuse service to any pet exhibiting aggressive behavior or posing a threat to the safety of other animals, clients, staff, or property. 


By submitting your payment to us you acknowledge that you have read and agreed to all of the above conditions and fees.
